You can register with this practice if you live in the practice area (inside the black line). ID will be requested if necessary. The practice area covers parts of NE2, NE3, NE4 and parts of NE6 and NE12. Please view the map below to see the area that we cover.
The Practice welcomes new patients. We will request details of your past medical problems so as to avoid delay while we wait for your records to reach us. Hand written records can take anything up to three months to arrive at the practice.

If you wish to register with us you need to complete the following:
- GMS1-Form (forms also available at the practice)
- New-Patient-Questionnaire (forms also available at the practice)
You will also need to bring with you the following pieces of documentation. This is to try and help to prevent fraudulent registrations. We will not refuse you registration if you are unable to produce documentation, but it may delay the registration process:
- Photo ID
- Proof of Address
All patients when registering with the practice are required to make use of the Height, Weight and Blood Pressure machine located in the lobby (for free). It can sometimes take a while for medical information to come through from your last practice and this allows us to start the process of updating your information.
If you have any long term conditions or are on long term medication you are encouraged to book an appointment with one of the GPs so that we can ensure appropriate reviews are in place whilst waiting for your medical records to arrive.
Register before you need us
Many new patients only register with us when they have a medical problem. We strongly recommend that you register when you move into the Practice area. This advice applies particularly to students who often forget to register with a doctor when they arrive at college. You have the right to express a preference to see a particular doctor and we will endeavour to ensure that you are registered with the doctor of your choice
Patient Choice Scheme for Out of Area Registrations
We are not participating in the Patient Choice Scheme for Out of Area Registrations. This is for a number of reasons, the most important of which is that we have not yet been informed of what the urgent care arrangements will be for out of area patients who are unable to attend the surgery. We believe that this may impact on patient safety so we will continue to request that patients moving out of our practice area reregister with a new practice.
