Holiday Vaccination forms – please save this form and email to [email protected]

If you are going aboard and need to know what injections are needed for the countries that you are visiting, then an early visit to the practice nurse may save you time and money and even illness, which is what the vaccination is all about. You will be asked to fill in a form, which will be handed to the practice nurse before you see her.

Even if you book a late holiday, then she can still give you advice about accelerated courses of vaccination which may have to be completed on your return but will give some cover. She can also give you advice regarding malaria prophylaxis and general advice. We have an internet connection to a web site, which is updated daily with advice about malaria prophylaxis and other requirements for your visit. In general vaccinations for foreign travel are no longer free on the NHS and there will be a charge payable at the time of appointment.  Please look at the page for non NHS charges as some of the vaccines such as rabies are very expensive.
